Eldercare Team Leader
Yokai is a leading Aboriginal community-controlled organisation committed to healing, advocacy and improving outcomes for Stolen Generations survivors, their families and communities.
We are seeking an experienced, organised and compassionate leader to coordinate our growing Eldercare Program.
This is an opportunity to lead a dedicated team that supports Aboriginal Elders to navigate the aged care system, access essential services, advocate for their rights and remain connected to community with dignity and respect.
As the Eldercare Team Leader, you will provide day-to-day leadership of our Eldercare Connectors, ensuring high-quality, culturally safe and trauma-informed services are delivered consistently across the program.
Working closely with the Eldercare Manager, you will oversee referrals, supervise staff, coordinate client allocations, monitor service quality and help drive continuous improvement across the program.
This is a hands-on leadership role for someone who enjoys developing people, creating efficient systems and ensuring every client receives outstanding support.
Key responsibilities
Lead, mentor and support a team of Eldercare Connectors.
Monitor workloads, case progression and program outcomes.
Provide guidance and support on complex client matters.
Ensure client documentation and case notes meet organisational and funding requirements.




Build and maintain strong relationships with My Aged Care, Aboriginal Community Controlled Health Services, Older Adult Health Hubs, GPs and community partners.
Identify opportunities to improve systems, processes and service delivery.
Foster a positive, accountable and collaborative team culture.
Promote trauma-informed, culturally safe and person-centred practice in all aspects of service delivery.
About you
You will be an Aboriginal and/or Torres Strait Islander Person.
You'll be someone who genuinely enjoys helping others succeed.
You are calm under pressure, well organised and naturally proactive.
You communicate well, build positive relationships easily and take pride in creating systems that allow teams to perform at their best.
You will ideally bring:
Experience leading or coordinating a team.
Strong organisational and time management skills.
Excellent communication and interpersonal skills.
Administration and computer skills.
Experience in community services, aged care, Aboriginal health or a related sector.
Understanding of trauma-informed and culturally secure practice.
A current C Class Driver's Licence.
