About the Role
Stream Services is seeking an experienced and motivated Service Delivery Manager to join our leadership team, based in Townsville.
This pivotal role oversees service delivery across multiple regional Queensland communities, ensuring the highest standards of care and operational excellence.
The position combines hands-on leadership with strategic oversight, working closely with teams to ensure compliance, drive innovation, and achieve outstanding outcomes for participants.
Frequent travel to rural and remote communities will be required.
Key Responsibilities:
Lead and manage support worker teams to deliver exceptional, person-centred services.
Ensure services comply with NDIS Practice Standards and meet individual participant goals.
Maintain strong relationships with participants, families, and stakeholders.
Oversee compliance, risk management, and incident reporting processes.
Drive participant engagement and contribute to business growth.
Provide regular reports on outcomes, performance, and financial metrics.
About You




Essential Skills & Experience:
Minimum 5 years' leadership experience in human services, preferably within the NDIS.
Strong understanding of NDIS service delivery models, including Supported Independent Living (SIL), restrictive practices, and plan utilisation.
Experience working in rural or remote settings, with cultural competence in diverse communities.
Proven ability to lead, motivate, and support multidisciplinary teams.
Excellent communication and interpersonal skills.
High proficiency in client management systems, reporting tools, and Microsoft Office.
Qualifications:
Relevant tertiary qualification in disability services, human services, social work, or a related field is highly regarded
