They know every participant by name.

The Role:
What is required in the role:
Lead care planning, individualised budgeting, and ongoing review for your participants under the NDIS program.
Be the dedicated point of contact for each participant and any registered supporter.
Hold regular care discussions with every participant on your caseload, at a minimum of one direct care management activity per month.
Set up service agreements, coordinate the right mix of supports, and manage participant budgets.
Apply a wellness and reablement approach, supporting independence and dignity of risk.
Work alongside their nursing team, admin team, and support workers to make sure services match goals.
Maintain accurate records and care management activity logs that meet the Quality Standards.
Coordinate access to related programs where participants are eligible.
What you need to bring to be Successful (Essential Criteria):
Relevant qualification in aged care, health, social work, nursing, a related field, or significant equivalent sector experience.
Working knowledge of the NDIS program, or recent experience as an NDIS Coordinator, Case Manager, or Care Partner.
Confidence with Government portals.
Demonstrable success in building your participant base.
Excellent interpersonal skills, judgment, and follow-through.
Current Australian driver's licence and reliable vehicle.




NDIS Screening or National Police check.
Genuine alignment with Platinum's HEART values.
