Based in Atherton, we are a community-driven not- for-profit organisation supporting people with disability to access the services and supports they need to achieve their goals.
Due to an increased demand within the program, we are seeking aPermanent Part Time Support Coordinatorto join our dedicated team and make a real difference in the lives of NDIS participants across the Tablelands region.
About the Role
As aSupport Coordinator, you will assist NDIS participants to understand, implement, and manage their NDIS plans.
You'll help people identify their goals, connect with service providers and build the capacity to exercise choice and control over their supports.
This role is ideal for someone with experience in disability, case management, or human services who thrives on building relationships and delivering high-quality, person-centred support.
Key Responsibilities
Support participants to understand, implement, and manage their NDIS plans.
Identify goals, strengths, and needs to develop tailored support strategies.
Coordinate supports across service providers and systems to ensure effective outcomes.
Facilitate capacity-building and self-management skills for participants.
Build and maintain strong relationships with participants, families, carers, and providers.
Act as a liaison between participants and mainstream/community services.
Ensure all documentation meets NDIS Practice Standards and organisational requirements.
Advocate for participant rights and access to appropriate services.
Work collaboratively with colleagues and contribute to continuous improvement.
About You
You are a compassionate, organised, and proactive professional who thrives in roles that combine coordination and collaboration.
With a deep understanding of the NDIS, you are dedicated to empowering people with disability to achieve their goals and enhance their independence.
You approach your work with empathy, professionalism, and a strong commitment to person-centred practice, ensuring that every individual is supported to live their best life.
Essential Criteria
Experience in disability support, case management, or a related human services sector.
Strong knowledge of the NDIS and person-centred, capacity-building approaches.
Excellent communication, negotiation, and problem-solving skills.
Ability to manage multiple participants and coordinate complex service networks.
Proficiency in Microsoft Office and client management/reporting systems.
Current First Aid certificate (including CPR).
Working with Children Check (Blue Card) and NDIS Worker Screening (Yellow Card).
Current, unrestricted QLD Driver's Licence.
Right to work in Australia.
Desirable
Qualifications in Social Work, Disability, or Community Services (or equivalent).
Experience supporting people with psychosocial disabilities or complex needs.
Experience with working within culturally safe practice, particularly when working with Aboriginal and Torres Strait Islander peoples.
