Clinical Care Coordinator

Lead safe, person-centred clinical care
Nexia Aged Care is seeking an experiencedClinical Care Coordinatorto join our clinical leadership team in New Gisborne.
Reporting to the Clinical Services Manager, you will coordinate the day-to-day delivery of clinical care and provide leadership, supervision and support to Registered Nurses, Enrolled Nurses and Personal Care Assistants.
You will act as the first point of escalation for resident deterioration, complex clinical needs and other clinical concerns.
Key responsibilities
Coordinate the daily delivery of safe, evidence-based and person-centred care.
Provide clinical leadership, guidance, coaching and support to nursing and care staff.
Oversee resident assessments, reassessments, care planning and clinical documentation.
Complete and oversee scheduled care-plan reviews within required timeframes.
Update care plans following incidents, changes in condition, hospital transfers or identified clinical risks.
Conduct clinical rounds and review residents with complex or high-risk care needs.
Identify, assess and respond appropriately to resident deterioration.
Monitor clinical risks, including falls, wounds, continence, nutrition and hydration, medication management, infection prevention, behaviours of concern, restrictive practices and palliative care.
Review incident reports to ensure assessments, investigations, notifications, open disclosure and follow-up actions are completed.
Finalise and close incident reports following a detailed review of all required information and actions.
Monitor outstanding incident actions and escalate concerns to the Clinical Services Manager.
Support SIRS identification, reporting, investigation and follow-up.
Coordinate admissions, transfers, discharges, referrals and multidisciplinary care reviews.
Liaise effectively with residents, representatives, GPs, allied health professionals and external providers.
Participate in audits, continuous improvement and regulatory-readiness activities.
Support AN-ACC documentation and accurate clinical resource allocation.
About you
You will have:
Current registration as a Registered Nurse with AHPRA.
A minimum ofthree years' full-time equivalent post-registration clinical experience.
Demonstrated residential aged-care experience, including clinical leadership, care coordination and supervision of multidisciplinary teams.
Strong clinical assessment, care-planning, documentation and decision-making skills.
Knowledge of the Aged Care Act ****, Strengthened Aged Care Quality Standards, Statement of Rights and Aged Care Code of Conduct.
Experience managing clinical incidents, SIRS requirements, clinical risks and open disclosure.
Demonstrated ability to identify and respond to resident deterioration and complex care needs.
Experience supervising, mentoring and developing Registered Nurses,
Enrolled Nurses and Personal Care Assistants.
Solid written and verbal communication skills.
Ability to manage competing priorities and changing operational requirements.
Experience using electronic clinical, care-management and medication-management systems.
Current First Aid and CPR certification.
Current National Police Check suitable for aged-care employment.
Current NDIS Worker Screening Check.
Evidence of compliance with applicable vaccination requirements.
No current Aged Care or NDIS banning order.
Desirable
Postgraduate qualifications in aged care, clinical nursing, gerontology, leadership or management.
Previous experience as a Clinical Care Coordinator, Nurse Unit Manager or Clinical Lead.
Experience with AN-ACC, regulatory assessments and quality-improvement activities.
Additional experience in wound management, infection prevention, dementia care, continence or palliative care.
Experience delivering workplace education and clinical mentoring.
