We are seeking an experienced and compassionateCare Partnerto join our Community Aged Care team.

About the Role
As a Care Partner, you will manage a portfolio of clients receiving government-funded aged care services.
Using a person-centred approach, you will work closely with clients, families, service providers, and internal teams to ensure services align with client goals, needs, and the strengthened Aged Care Quality Standards.
Your role will involve conducting home visits, coordinating support services, monitoring client wellbeing, managing risks, and responding proactively to changes in client circumstances.
Key Responsibilities
Manage and coordinate a caseload of clients receiving Support at Home services
Develop, review and update care plans to ensure client goals and support needs are met
Conduct regular home visits, assessments and reviews
Monitor client wellbeing and respond promptly to changes in needs
Work collaboratively with families, representatives, clinical teams and external stakeholders
Identify, assess and manage client risks
Support quality service delivery through effective communication with rostering and support worker teams
Investigate and respond to client feedback and complaints
Complete accurate case notes, reports and compliance documentation
Participate in service improvement initiatives and quality assurance activities
Provide supervision and performance reviews for Support Workers
Participate in an after-hours on-call roster
About You




You are a proactive and organised skilled who is passionate about delivering positive outcomes for older people.
You can effectively manage competing priorities while maintaining a high level of client care and compliance.
To be successful, you will have:
Relevant qualifications and/or demonstrated experience in aged care
Experience coordinating community-based support services
Knowledge of aged care programs and quality standards
Strong administration, organisation and time management skills
Excellent written and verbal communication skills
A customer-focused and solution-oriented approach
Experience working with a broad range of stakeholders
Proficiency in Microsoft Office applications
Essential Requirements
Current unrestricted Driver's Licence
Current First Aid Certificate
NDIS Worker Screening Check (or ability to obtain)
Ability to undertake regular travel and home visits
Availability to participate in an after-hours on-call roster
