We are looking for an experienced and
compassionate Senior Social Worker to join our team within the
Fremantle Older Adult Mental Health Service.
This is a full-time role, available
on either a permanent basis or fixed-term 12-month contract,
with the possibility of extensions and/or permanency.
As part of a consumer-focused,
collaborative multidisciplinary team, you will provide advanced
clinical Social Work services for older adults experiencing mental
health concerns.
In this role, you will
also:
Deliver clinical care:Plan, implement,
coordinate, and evaluate recovery-oriented, trauma-informed,
and positive-aging services for consumers and their
families.
Ensure safety:Plan, execute, and evaluate
consumer safety initiatives and quality of care.
Lead a team:Supervise Social Work staff,
students, and support personnel, while coordinating daily
duties and managing performance.
Maintain professional standards:Ensure
all practice aligns with the Australian Association of Social
Workers Practice Standards **** and Code of Ethics **** and
SMHS policies and guidelines.

Peel and Rockingham Kwinana Mental Health (PaRK Mental Health),
specialist services are provided across the South Metropolitan
region, supporting consumers with accessible, collaborative and
high-quality mental health care.
SMHS Mental Health
will soon open a new state-of-the-art facility at Fremantle
Hospital designed to offer expanded and contemporary mental health
services for our community.
The three-storey space will provide an
additional 40 mental health beds and a 24-hour Mental Health Crisis
Centre, ensuring compassionate and innovative care for those who
need it most.
