Care Partner – Support at Home About UsYNA, in partnership with Alpha Support at Home (ASAH), delivers high-quality aged care and community-based services across Australia.
Together, we support older Australians to live independently, safely, and with dignity through person-centred, compliant, and outcomes-focused care.
About the RoleAs a Care Partner – Support at Home, you will be responsible for care management activities for older people receiving services under government and private funding.Each participant is allocated a dedicated Care Partner who works collaboratively with them to plan, manage, and coordinate services aligned to their aged care assessments, funding, and individual goals.
Where appropriate, and with participant consent, you will involve informal carers, supporters, and family members in the care planning process.
Participants with complex needs may have their care overseen in collaboration with a clinical care partner.This role also contributes to intake processes, home visits, and timely onboarding of recent Alpha Support at Home clients across your allocated region.
Key ResponsibilitiesCare Management & Service CoordinationDeliver person-centred care planning aligned to participant goals, preferences, and assessed needsSupport participants to understand their approved services and funding, enabling informed decision-makingEnsure care plans promote reablement, independence, wellbeing, and dignity of riskMonitor, review, and adjust services and budgets in response to changing participant needsMaintain regular participant contact,



including a minimum of one care management activity per participant per monthConduct home visits for care planning and review purposes as requiredRefer participants to My Aged Care for reassessments and support plan reviews when appropriateEnsure all care management activities align with Standard 3 of the Aged Care Quality Standards
Intake & Service CoordinationCoordinate intake and onboarding of new clients in a timely mannerMonitor the client waitlist and manage admissions efficientlyLiaise with multiple service providers to ensure timely, high-quality service deliveryCoordinate and manage participant care planning against budgets within approved funding levelsIdentify and escalate client and financial risks to the clinical and finance teams respectivelyWork closely with internal Program Support, Finance, and Administration teams to resolve participant enquiries
About YouExperience working in health, aged care, or community-based settingsAHPRA Registered Nurse with aged and/or community-related fieldStrong understanding of person-centred care and aged care funding modelsExcellent communication, coordination, and stakeholder engagement skillsAbility to manage competing priorities while maintaining compliance and quality outcomes
