The position involves providing day-to-day support to clients in the community, including assisting with daily living activities, promoting independence, and encouraging participation in social and community programs.
The role includes supporting clients to access relevant services, accompanying them to appointments, and helping them navigate local health and social supports.
The Community Support Worker will document client progress, collaborate with colleagues and external providers,



and contribute to individualized support plans.
The role also requires maintaining professional boundaries, following organizational policies, and upholding safety and quality standards in all interactions.Qualifications Candidates should possess robust Interpersonal Skills and Communication abilities to build rapport and maintain effective, respectful relationships with clients and colleagues.Candidates should possess Community Support and Community Health skills to assist individuals in accessing services, promoting wellbeing, and encouraging participation in community activities.Candidates should possess Case Management skills to help coordinate supports, monitor client progress, and maintain accurate documentation.Relevant qualifications in community services, social work, disability support, or a related field are beneficial.Experience working with diverse communities, including people with disability, mental health conditions, or social disadvantage, is advantageous.Ability to work independently and as part of a multidisciplinary team, with sound time management and problem-solving skills.Knowledge of local community resources and support services, and a commitment to person-centered and trauma-informed practice.
