The Support Coordinator will support participants to search for, coordinate and implement all supports in their NDIS plan, including informal, mainstream, community and funded services and resources.
In this role, you will actively support participants to build on their skills and work with them to increase their ability to access relevant services within their local community and achieve greater independence.
My Abilities provides NDIS Support Coordination services to participants with a wide range of needs and complexities.
We work collaboratively with clinicians, therapists, and other professionals to ensure the delivery of comprehensive and effective support.
Our approach is holistic, focusing on achieving the best possible outcomes for each participant.
We are committed to person-centred practice, ensuring that supports are tailored to individual goals and circumstances.
By establishing appropriate and sustainable supports, we aim to build the capacity of both participants and their stakeholders.
We prioritise choice and control, delivering flexible and creative support that responds to the unique needs of each individual.
Key Responsibilities:

· Implement and support the effective delivery of participants' NDIS plans.
· Collect relevant evidence and prepare high-quality reports, including detailed progress reports and narratives.
· Work with individuals with diverse abilities and varying levels of complexity, ensuring appropriate and responsive support.
· Connect and coordinate supports by engaging service providers best suited to meet participants' needs.
· Facilitate access to appropriate services, including Occupational Therapy, Speech Pathology, Physiotherapy, Community Access, and recreational activities.
· Manage service agreements, service bookings, and ensure all NDIA reporting requirements are completed accurately and within required timeframes.
· Support participants in exploring and identifying their goals in line with their NDIS funding.
· Promote capacity building by strengthening participants' skills and independence within their community.
Monitor, review, and evaluate participants' progress towards their goals, ensuring supports remain effective and aligned with outcomes.
