Latrobe Regional Health is looking forHaematology Clinical Nurse Coordinatorto join our growing team on apart time (0.2EFT) ongoing basis.
The Haematology Clinical Nurse Coordinator provides specialist clinical coordination, support and education for patients diagnosed with haematological conditions across their care journey.
Working within the Chemotherapy Service, this role contributes to improved patient outcomes through care coordination, clinical liaison, supportive care and collaboration with multidisciplinary teams and external health partners.
The position plays a key role in facilitating continuity of care across inpatient and outpatient settings, supporting transplant and CAR ‐ T therapy pathways, coordinating follow ‐ up care and contributing to service development and quality improvement initiatives.

The role of theHaematology Clinical Nurse Coordinatoris to:
Coordinate and provide specialist support, education and care navigation for patients with haematological conditions across the continuum of care.
Liaise with inpatient, outpatient and acute care teams to support treatment planning, transitions of care, planned admissions and patient follow ‐ up.
Coordinate transplant and advanced treatment pathways, including autologous, allogeneic and CAR ‐ T therapy patients in collaboration with partner organisations.
Participate in multidisciplinary meetings and coordinate patient management activities, including post ‐ MDM follow ‐ up and ongoing supportive care programs.
Contribute to staff education,



policy development, quality improvement initiatives and continuous enhancement of haematology services.
About you
Ideal candidates for this role will have:
Current registration as a Registered Nurse with AHPRA.
At least three years' experience in haematology and/or oncology nursing, or a relevant postgraduate qualification.
Demonstrated clinical expertise in coordinating complex patient care and supporting patients through specialised treatment pathways.
Strong organisational, problem ‐ solving and care coordination skills, with the ability to manage multiple priorities.
A commitment to patient ‐ centred care, professional development, staff education and continuous quality improvement.
Postgraduate studies in Haematology Nursing and additional experience in specialist haematology services, transplantation pathways or oncology care (desirable).
