We are recruiting a Registered Nurse who will work in a small, collaborative clinical team to deliver high-quality clinical care and care coordination across our community programs.
This role will be based in the Illawarra region, with occasional travel to the Shoalhaven region required.
Payrate:$*****/hour + NFP Salary Packaging + KM reimbursement
Hours per week:Full time, 38 hours/ week.
We are open to a part-time role for the right candidate.

Role highlights

What you'll be doing
Deliver direct, comprehensive nursing care and individual case management to aged care and NDIS participants in their homes and community
Conduct nursing assessments and develop, implement and review car plans and Advanced Health care plans in collaboration with clients and their families/ carers.
Support direct care workers through education and guidance to maintain best practice clinical standards
Coordinate services across health providers and community stakeholders to meet participants changing needs.
What we're looking for
Essential
Registered Nurse with current AHPRA registration
Demonstrated experience working with complex clients in aged care, disability, or community settings
Demonstrated clinical leadership and mentoring capability
Current NSW Driver's Licence
Current First Aid Certificate
Desirable (highly regarded)
Advanced clinical assessment and case management experience
Knowledge of Home Care Packages (HCP) and Support at Home program
Experience in palliative care, dementia, or end‐of‐life care
Understanding of Aged Care Quality Standards and clinical governance
