Lead meaningful lifestyle, wellbeing and resident engagement programs
Nexia Aged Care is seeking a creative, organised and compassionateLifestyle Coordinatorto lead our lifestyle and wellbeing services in New Gisborne.
Reporting to the Residence Manager, you will plan, coordinate, implement and evaluate a comprehensive lifestyle program that reflects residents' assessed needs, preferences, goals, cultures, spirituality, abilities and life experiences.
You will support residents to remain physically, socially, emotionally, cognitively and spiritually engaged while promoting dignity, choice, independence and supported decision-making.
Key responsibilities
Plan, coordinate and evaluate a comprehensive seven-day lifestyle program.
Complete lifestyle assessments and develop individualised lifestyle care plans.
Deliver meaningful engagement opportunities for all residents.
Support residents living with dementia, cognitive impairment, sensory loss or complex care needs.
Coordinate individual, small-group and large-group activities.
Plan outings, celebrations, themed events, entertainment and intergenerational activities.
Encourage resident participation, independence and community involvement.
Build partnerships with families, volunteers, schools and community organisations.
Prepare monthly lifestyle calendars, newsletters and promotional materials.
Monitor attendance, resident participation, satisfaction and program outcomes.
Supervise and support Lifestyle Assistants, volunteers and students.
Coordinate orientation and induction for new lifestyle employees and volunteers.
Manage lifestyle resources, equipment and approved budgets.
Maintain accurate progress notes, attendance records and program evaluations.
Respond to feedback and contribute to audits and continuous improvement.
Ensure lifestyle services comply with aged care legislation and organisational requirements.
About you
You will have:
A Certificate IV, Diploma or Degree in Diversional Therapy, Leisure and Health, Recreation, Lifestyle or a related discipline.
A minimum oftwo years' experiencecoordinating lifestyle, leisure or wellbeing programs within aged care, health or community services.
Robust knowledge of person-centred care and resident-directed lifestyle planning.
Experience completing lifestyle assessments and developing individualised lifestyle care plans.
Demonstrated leadership and supervisory skills.
Excellent communication and relationship-management abilities.
Strong organisational, time-management and problem-solving skills.
Confidence using Microsoft Office and electronic care-management systems.
A current National Police Check suitable for aged care employment.
A current NDIS Worker Screening Check.
A current Victorian Driver Licence and the ability and willingness todrive the facility bus, subject to licence and organisational requirements.
Evidence of compliance with applicable immunisation requirements.
No current Aged Care or NDIS banning order.
Experience delivering dementia-specific, culturally inclusive, creative or therapeutic programs will be highly regarded.
