About This Role
Working closely with our Case Managers and multidisciplinary team, you'll play a key role in coordinating safe, seamless and patient-centered transitions of care.
As a Discharge Planner, you'll work with patients, families and external healthcare providers to develop personalised discharge plans that support each patient's goals and ongoing care needs.Hours/Employment
Permanent Part Time 0.4 FTE
Key Responsibilities
Develop personalised discharge and continuing care plans tailored to each patient's individual needs.
Coordinate transitions of care by liaising with patients, families, carers and external providers, including rehabilitation, aged care and palliative care services.
Advocate for patients to ensure their physical, emotional, cultural and healthcare needs are considered throughout the discharge planning process.
Provide education and resources to patients, carers and staff to support safe and informed care transitions.
Collaborate with the multidisciplinary team to facilitate timely discharge planning and continuity of care.
Maintain accurate documentation in accordance with hospital policies and professional standards.
About You
Experience in discharge planning, case management or care coordination within a healthcare setting.
Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, families and healthcare providers.
Strong organisational and problem-solving skills, with the ability to manage competing priorities.
A collaborative approach and commitment to delivering high-quality, patient-centred care.
Current AHPRA registration
