The Trustee for Yes Disability Services Trust About Us YES Disability Services is a trusted provider of quality in-home care and disability support services.
We are committed to delivering compassionate, person-centred care that enables our clients to live safely, comfortably, and independently. We are currently seeking caring and dedicated Personal Care Workers to join our growing team. Key Responsibilities Provide respectful and person-centred personal care, including assistance with showering, dressing, grooming, toileting, and personal hygiene while maintaining clients' dignity and privacy.
Assist clients with mobility, transfers, positioning, and daily living activities to promote independence and wellbeing. Support clients with meal preparation, feeding, and ensuring their nutritional requirements are met in accordance with individual care plans. Provide companionship, emotional support, and encourage clients to maintain social connections and participate in community activities.
Perform light household duties, including cleaning, laundry, and maintaining a safe, comfortable, and hygienic home environment. Assist clients with rehabilitation exercises and therapy programs as directed by healthcare professionals. Provide medication assistance in accordance with organisational policies and procedures.




Monitor and report any changes in clients' physical, emotional, or behavioural condition to supervisors and care coordinators. Follow individual care plans and organisational policies to deliver high-quality care and support. Work collaboratively with families, healthcare professionals, and other members of the care team to ensure the best outcomes for clients.
Essential Requirements Certificate III in Individual Support (Ageing, Disability or Home and Community) or equivalent qualification. Previous experience in personal care, aged care, disability support, or home care is preferred. Excellent communication and interpersonal skills.
Compassionate, reliable, and committed to providing high-quality person-centred care. Current First Aid and CPR Certificate.
Current NDIS Worker Screening Check. Current Working with Children Check (where applicable).
Current Australian Driver's Licence and reliable vehicle preferred. What We Offer Attractive salary package with Superannuation.
