Job Description

Ability Action Australia is growing — and we're looking for a Behaviour Support Practitioner to join our multi-disciplinary team on the Sunshine Coast.

The successful candidate will be providing supports to NDIS participants living close to the Glasshouse Mountains area.

Responsibilities Will Include

Assisting NDIS participants, along with their families and/or carers, to manage challenging behaviours to improve their quality of life, and develop current skillsYou will complete thorough assessments to understand behaviour triggers, identify risks & consider specific strategies to improve behaviour outcomesYou will develop Behaviour Support Plans and provide support to families and/or carers, for consistent implementation Maintain a caseload with ongoing monitoring, support and make adjustments where required.Develop meaningful goals and practical strategies utilising a positive behaviour support framework, with a commitment to eliminate restrictive practices, in the home and communityEnsuring individual plans reflect our NDIS participants' goals, passions and strengths, as well as ensuring they get more out of lifeBuild the knowledge,



capacity and strengths of those who support participants in their lives - family, carers or friendsAct as an advocate and role model for best practiceWorking collaboratively within a multidisciplinary team
