About the role
The Returned & Services League of Australia (S.A. State Branch) is seeking a full-time Care Coordinator to join our team in Adelaide, SA. In this crucial role, you will be responsible for coordinating and delivering high-quality care and support services to veterans, serving members of the Australian Defence Force, and their families. Your work will have a direct and meaningful impact on the lives of those who have served our country.
What you'll be doing
Develop and implement individualised care plans to address the unique needs of each client
Facilitate connections between clients and trained RSL Support Officers
Liaise with a range of health and community service providers to coordinate the delivery of tailored support services
Regularly monitor and review client progress, adjusting care plans as needed
Maintain detailed case notes and records in accordance with organisational policies and procedures
Provide emotional support and practical assistance to clients and their families
Advocate on behalf of clients to ensure they receive the care and services they require
Participate in team meetings and contribute to the continuous improvement of care processes
What we're looking for
Relevant qualification in a field such as social work, nursing, or community services
Experience in a care coordination or case management role, preferably within the community services or healthcare sectors.




An understanding of the NDIS would be advantageous
Excellent communication and interpersonal skills, with the ability to build trusted relationships with clients
Strong problem-solving and decision-making abilities, with a focus on client-centred outcomes
Proficiency in documenting and reporting on client progress and service delivery
