As our services continue to grow across Tasmania, we are seeking an experienced and motivated
NDIS Support Coordinator and Psychosocial Recovery Coach
to join our dedicated team in Hobart.
About the Role
As an NDIS Support Coordinator and Psychosocial Recovery Coach, you will work closely with participants to help them understand and implement their NDIS plans, build their capacity, connect with appropriate supports, and achieve meaningful outcomes.
You will collaborate with participants, families, carers, allied health professionals, community organisations, and service providers to ensure participants receive coordinated, high-quality support tailored to their individual needs.
Key Responsibilities
Deliver Support Coordination and/or Psychosocial Recovery Coaching services to NDIS participants.
Assist participants to understand their NDIS plans, funding categories, and available supports.
Support participants to identify goals and develop practical strategies to achieve them.
Coordinate supports with allied health professionals, service providers, and community organisations.
Promote participant choice, independence, and decision-making.
Monitor participant funding utilisation and support effective plan implementation.
Prepare accurate case notes, progress reports, and review documentation.
Build positive relationships with participants, families, guardians, and stakeholders.
Identify and manage risks while escalating concerns in accordance with organisational policies.
Maintain compliance with NDIS Practice Standards, organisational policies, and relevant legislation.
Essential Requirements
Previous experience working within the NDIS sector.
Demonstrated experience in Support Coordination and/or Psychosocial Recovery Coaching.
Strong understanding of the NDIS, funding categories, and participant-centred practice.
Excellent communication, interpersonal, and stakeholder engagement skills.
Strong organisational, time management, and case management abilities.
Ability to work independently while managing multiple participants and competing priorities.
Current NDIS Worker Screening Check.
Current Working with Vulnerable People Registration (Tasmania).
Current Australian Driver Licence and reliable vehicle.
Excellent written documentation and report-writing skills.
Desirable Qualifications
Certificate IV or Diploma in Community Services.
Mental Health qualifications or Psychosocial Recovery Coach experience.
Qualifications in Social Work, Psychology, Counselling, Occupational Therapy, or related disciplines.
Knowledge of local community and mainstream support services.
