About the Role
The overall purpose of the
Practitioner, Foster & Kinship Care
is to recruit, train, assess and support Foster & Kinship carers to meet the individual needs of children and young people in out of home care (OOHC).
The Practitioner, Foster & Kinship Care will ensure that a targeted level of support is provided to all care placements, based on identified needs and program type.
Depending on the program model, you may also provide therapeutic interventions, direct support and coordinate external services to enhance placement outcomes.
This role is ideal for someone passionate about making a meaningful difference in the lives of vulnerable children, young people and families.

fixed-term, Full time
position based in
Nerang
until
31 st of Dec ****.
The salary for this position starts from
$***** per hour
, plus superannuation and access to salary packaging benefits.
Responsibilities
Recruit, screen, assess and support Foster and Kinship Carers throughout the approval process.
Deliver and coordinate training programs for prospective and existing carers.
Conduct comprehensive carer assessments and renewals to identify strengths, risks and support needs.
Provide ongoing support, supervision and guidance to Foster and Kinship Carers.




Conduct regular home visits and maintain consistent contact with carers to monitor placement wellbeing.
Collaborate with stakeholders to identify and coordinate appropriate care placements and support services for children and young people.
Facilitate therapeutic and targeted interventions for placements requiring enhanced or intensive support.
Promote and support family and kinship connections, including reunification planning where appropriate.
Maintain accurate records and ensure compliance with licensing, legislative and organisational requirements.
Work collaboratively with internal teams and external agencies to deliver child-centred, high-quality care outcomes.
Qualifications, Credentials and Other Requirements
Degree in Social Work, Psychology, Human Services, Behavioural Science or similar field
Current QLD Drivers' Licence & own reliable vehicle
Current Paid Blue Card
Current Licensed Care Service (LCS) or willing to apply
Willingness to travel within service region
