Part‐time or Full‐time positions at St Vincent's Care at Home (SVCAH), located in Sydney Private Hospital, Darlinghurst.
Responsibilities
Work collaboratively with a close‐knit multidisciplinary team, including MCAH Leads, Visiting Medical Officers, St Vincent's Care Facilities and other allied health staff.
Assess patient needs, develop tailored care plans, and coordinate safe discharge.
Deliver advanced nursing interventions, including IV therapies, complex wound care and monitoring of complex conditions.
Manage and coordinate new referrals, escalating care when required.
Promote patient independence and enhance quality of life, strengthening the bridge between hospital and home care.
Provide guidance and mentorship to new staff members within the growing team.
Qualifications
Registered Nurse with AHPRA.
At least 3 years post‐graduate nursing experience in community, medical‐surgical or critical care settings.
Strong interpersonal and communication skills to build rapport quickly.
Availability to work a variety of shifts, including weekends and overnight on‐call (telephony only).
NSW driver's licence with no restrictions.
Full and unrestricted working rights in Australia.
