About us
Brisbane South PHN is one of 31 regional Primary Health Networks (PHNs) established nationally by the Australian Government to help deliver an productive and effective primary health care system for the people of Australia.
We work with health professionals and communities to understand our region's health and wellbeing needs, and commission services that respond to those needs. By investing in person-centred, place-based models of care, we deliver results for the people and communities in our region and support broader health system reform.
About the role
Join Brisbane South PHN's General Practice Quality Improvement team and help strengthen primary healthcare across our region. In this role, you will coordinate and implement quality improvement programs that support general practice teams to use data, evidence and practical improvement methods to improve patient health outcomes. You will coach and guide practices, build strong partnerships across the primary healthcare sector, contribute to strategic planning, and support others to deliver meaningful impact.
This is a full-time, fixed-term role until 30 June **** (part time options would be considered).
In this position, you will:
Support general practice teams to apply evidence-based continuous quality improvement approaches, with a focus on population health and prevention priorities.
Work with primary health care providers to turn data into meaningful insights and practical actions that drive change, continuous improvement and person-centred care.
Contribute to the achievement of General Practice Quality Improvement team goals and support Brisbane South PHN's strategic goals and vision.
About you




To be successful in this position, you will have the following:
Relevant qualifications in health, business or a related field, or equivalent experience, along with a strong understanding of primary health care, general practice and the Australian health system.
Confidence working with data, digital tools and reporting systems to turn information into practical insights, actions and measurable improvement.
Hands-on experience applying quality improvement methods to support meaningful, evidence-based change in primary health care settings.
A collaborative, relationship-focused approach, with clear communication skills and the ability to engage stakeholders, tailor your message and contribute to a supportive team culture.
