Community Registered Nurses - Palliative Care

About The Role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client's home or residential accommodation.

Key Responsibilities

Support and education for clients with terminal diagnosesEducation and support for families and carersAdministration and preparation of s/c medications and CSCI for symptom managementGeneral nursing care including simple and complex wound care, IDC/SPC care, PICC dressingsAccurate/ Timely documentation in all client records.Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.Complete initial assessments/develop care plans to ensure best outcome for our clients.Monitor and update care plans to ensure appropriate care is providedRecognise and respond to clinical deterioration, with appropriate escalation.Promoting best practice and initiating quality improvement activities

What You Bring





Current AHPRA NursingMinimum 2 years palliative care nursing experience, community experience desirablePost graduate qualifications (or working towards) in Palliative CareNDIS Clearance (or willingness to obtain one)Police Clearance with in 6 months (or willingness to obtain one)Use of own car – KM allowance providedFull drivers licence, reliable and comprehensively insured carCPR and Manual handlingA desire to join a team committed to the care of others in a mission-based organisation
