About the Role
On behalf of our client, a registered NDIS provider dedicated to empowering individuals to "live well," we are seeking a dedicated Community Worker – Disability. Our client specializes in Supported Independent Living (SIL), Short Term Accommodation, Community Access, Allied Health/Therapeutic Support, and Behaviour Support for children and adults with disabilities or developmental delays. Their focus is on building daily living skills, fostering community participation, and providing a "home away from home." We are looking for a worker who will assist clients to solve social, emotional, and practical problems while promoting genuine independence.
Key Duties (paraphrased & detailed)
Assessment & Program Development
Assess clients' needs, abilities, and personal goals, then plan, develop, and implement individualised educational, training, and support programs (e.g., daily living skills, community access, therapeutic activities).
Interview clients and their families to determine the nature and extent of social, emotional, financial, or behavioural difficulties, using this information to tailor NDIS-funded supports.
Monitoring & Referral
Monitor and report regularly on clients' progress toward their SIL or community participation goals, adjusting support strategies as needed.
Refer clients to additional agencies or allied health professionals (e.g., behaviour support practitioners, community nurses)
when specialised help is required.
Community & Resource Liaison
Assess community needs and available resources for health, welfare, housing, employment, training, and other disability-specific facilities, then connect clients accordingly.
Liaise with community groups, government bodies, private businesses, and other welfare agencies to promote awareness of disability resources and advocate for client inclusion.
Direct Support & Family Assistance
Support families and provide education, personal care, and social engagement for disabled persons in adult service units, group housing, or short-term accommodation settings.
Assist young people and adults to solve social, emotional, and financial problems, including budgeting, accessing transport, or managing challenging behaviours using tailored behaviour support plans.
Administration & Funding
Prepare clear, timely submissions for funding and resources, including reports to government bodies (e.g., NDIA) and other agencies, ensuring compliance with NDIS Practice Standards.
Skills & Experience Required
Minimum 1 year full-time relevant experience (disability support, community services, or allied health assistance).
Relevant qualification (e.g., Certificate IV/Diploma in Community Services, Disability, or Mental Health).
Current NDIS Worker Screening Clearance (preferable), satisfactory police check, valid driver's license, and own transport.
Up-to-date flu vaccination and COVID-19 immunization (as required).
