CenterWell Home Health is seeking a compassionate and skilled Occupational Therapist, Home Health to deliver high-quality therapeutic care to patients in the comfort of their own homes. In this role, you will provide person-centered occupational therapy services focused on restoring function, maximizing independence, and improving overall quality of life for adult and geriatric patients. Working within the Healthcare & Medical Services department, you will conduct comprehensive assessments, develop and implement individualized treatment plans, and provide ongoing interventions that help patients safely perform activities of daily living, manage chronic conditions, and achieve their functional goals. You will collaborate closely with nurses, physical therapists, speech therapists, social workers, physicians, patients, and caregivers to coordinate holistic, evidence-based care.



Responsibilities include evaluating home safety and recommending adaptive equipment or environmental modifications, educating patients and families on therapeutic exercises and energy conservation strategies, documenting care in accordance with regulatory and agency standards, and participating in interdisciplinary case conferences to support optimal outcomes.
