About the Role Use your skills to advocate for older people facing complex geriatric needs who are living in the community. As a Social Worker within Integrated Care Geriatric Services, you will provide comprehensive assessments, support and short-term case management to help clients navigate complex health needs, social challenges and services.

About You You are a compassionate Social Worker with sound clinical judgement who is passionate about supporting older people through holistic, person-centred care.
Social work degree and eligible for AASW full membership
Manage complex geriatric caseloads with confidence
Deliver holistic assessments and case management
Build strong partnerships across health and community services
Advocate for older people and their wellbeing
