About the role:
We have
Permanent Part Time Night Shift

Roster 1
: Wk 1 & 2 – Thu to Sat – **** to **** (27hrs/wk)
Roster 2
: Wk 1 & 2 – Sun to Wed – **** to **** (36hrs/wk)
Provide dedicated leadership and supervision to Personal Care Workers and support Registered Nurses in their role to promote a harmonious team approach to the delivery of resident care at Uniting AgeWell –
Hawthorn
Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
Provide excellence in nursing care to residents with complex needs such palliative care, dementia support,



restorative care and pain management to enhance their wellbeing
To support nursing staff regarding assessment and care planning processes, documentation and evaluation of care outcomes
Analyse risk, implement strategies and participate in clinical handover with Registered Nurses
Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management in consultation with the Registered Nurse
About you:
You are passionate about what you do and want to make a difference in the lives of our residents to support them to live life to their potential!
Current registration with AHPRA as a Medication Endorsed Enrolled Nurse (Div 2)
Minimum 2 years experience as an Enrolled Nurse in residential aged care (must have)
Demonstrated experience with dementia care and palliative care support (desired)
Excellence in decision making, critical analysis and on the spot problem solving
Demonstrated abilities to delegate and prioritise in a quick paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
Available and committed to work in the above mentioned roster vacancies at Hawthorn - unsuitable for students with inconsistent availability

A current Working With Children Check employee card
Current and ongoing flu vaccinations
