About the client:
Our client provides a comprehensive assessment, treatment and assertive outreach support for individuals with moderate to severe mental health needs. As part of a multidisciplinary care team, you will contribute to a holistic, client centred model of care, integrating with regional health teams to ensure timely and appropriate care, including inpatient support when necessary.
Role Overview:
As a Social Worker, you will work within a collaborative multidisciplinary team, with mild, moderate and or severe mental health needs to ensure they are well supported in the community through a comprehensive model of primary health and mental health care, ensuring clients receive the right care at the right time.
Key Responsibilities:
Conduct culturally sensitive and thorough assessments of clients.
Provide treatment and intensive case management support.
Engage in assertive outreach to address emerging mental health needs.




Apply your advanced specialist knowledge and clinical expertise in mental health assessments and decision making.
Mandatory Qualifications & Experience:
Bachelor of Social Work with AASW registration
Willingness to undergo a Criminal History Check.
Current C-Class Driver's License.
Demonstrated high-level verbal and written communication skills.
At least 3 years of experience in a similar role.
