We are seeking a Registered Nurse to join our Community Health Care team and ensure the provision of wound care to community-based clients in the Penrith LGA.
This full‐time role operates Monday to Friday.
Uniform is required.
The successful incumbent will provide safe, effective secondary‐level nursing support to clients in the community.
This role forms part of both the Community Wound Care Team and the broader community nursing service, working under the direction of the Clinical Nurse Unit Manager.
In this position, you will deliver person‐centred care using a flexible, responsive, wellness and reablement approach aligned with the Australian Aged Care Quality and National Standards.




You will be responsible for the assessment, planning, implementation and evaluation of care using evidence‐based guidelines and Primary Health Care principles, working in partnership with clients, families and carers, as well as GPs, specialists and other key stakeholders.
You will provide support, advice and education to Community Nurses, Allied Health professionals, GPs, clients and carers, and participate in relevant Aged Care Quality and National Standards working groups.
The role may also require you to provide care across other sites and to other Community Health Centre clients as service needs arise.
Additionally, you will supervise Enrolled Nurses and delegate clinical tasks appropriately within their scope of practice.

What you will bring to the role
Current registration to practice as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA) with demonstrated experience and clinical skill in nursing as a Registered Nurse including completion of a new graduate Registered Nurse program or equivalent experience working as a Registered Nurse
Demonstrated skills and experience working in the provision of care to clients who have chronic and/or acute on chronic wounds
Demonstrated capacity to provide effective nursing care that is person centred, family inclusive and culturally sensitive to people accessing health services and an ability to liaise with other health professionals to facilitate case management and care planning.
Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously.
Demonstrated effective communication skills (written and verbal) and interpersonal skills.
Demonstrated commitment to professional development, evidence-based practice and an awareness of changes within the health care environment
Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
Current valid NSW State driver's licence (including P1 or above) and willingness to use for work purposes.
This may include driving long distances across the LHD.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
