Metanoia Supports is seeking an experienced and compassionate Family Support Worker to join our growing team.
We are looking for a highly organized, motivated and client-focused skilled who is passionate about supporting individuals and families to improve their wellbeing, strengthen their independence and access appropriate community services.
This is a full-time position of 38 hours per week.
Key Responsibilities
Conduct client assessments to identify individual and family support needs and assist in developing tailored support plans.
Provide practical guidance and ongoing support to individuals and families to achieve their personal and social goals.
Monitor client progress, maintain accurate case notes and prepare reports as required.
Assist clients in accessing appropriate community, health, education, housing and support services.
Liaise with community organisations, government agencies and external service providers to coordinate client support.
Provide information and education to families regarding available services, community resources and support options.
Support families in developing independent living skills, strengthening family relationships and improving overall wellbeing.
Identify barriers affecting clients and assist them in developing practical strategies to overcome social and personal challenges.
Participate in case reviews and contribute to the continuous improvement of client support programs.
Maintain professional relationships with stakeholders while ensuring services are delivered in accordance with organisational policies and relevant legislation.
Requirements
Minimum 1 year of relevant work experience in a similar role.
Certificate III in Individual Support or a relevant community services qualification.
Current NDIS Worker Screening Check.
Current National Police Check.
Current Blue Card.
Current First Aid Certificate.
Excellent communication, organisational and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Pay: $89,****** per year
