The Central Coast Local Health District (CCLHD) is currently seeking a dedicated and compassionate Registered Nurse to join our dynamic healthcare team within our Nunyara Aboriginal Health Unit.

About The Chance
The Registered Nurse position is an integral role within the Nunyara Aboriginal Health Unit, part of the Central Coast Local Health District (CCLHD).
This role focuses on supporting the health and wellbeing of Aboriginal patients and communities through culturally informed, patient‐centred care.
The Registered Nurse is responsible for ongoing monitoring of patient and community health needs, as well as responding to episodic health concerns for individuals referred to the Aboriginal Chronic Care service.
Working collaboratively within the Aboriginal Health Unit, the Registered Nurse will partner closely with Aboriginal Hospital Liaison Officers (AHLOs) and other team members.
The role provides clinical support, contributes to culturally safe care planning, and helps build strong internal and external partnerships to ensure comprehensive and culturally appropriate service delivery for Aboriginal patients accessing care within CCLHD.
Primarily,



this position provides case management, home visiting, and support for people living in the community with multiple chronic conditions who are at risk of deteriorating health.
The aim is to maintain quality of life, prevent avoidable hospital admissions, and ensure patients receive coordinated, holistic care.

Position Description
About You
As this is a targeted position that works directly with Aboriginal people, ideally, you are an Aboriginal person or a non-Aboriginal person with a sound understanding of providing culturally sensitive and trauma-informed care for Aboriginal people.
You have current Authority to practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
You have well developed communication skills and enjoy working in a multi-disciplinary team environment.
You have an understanding of the psycho-social impacts for Aboriginal and Torres Strait Islander patients with acute and chronic health conditions in a hospital and community social work setting
