The Sustaining NSW Families (SNF) program is a structured, home‐based antenatal and postnatal support service.
It is a nurse‐led, sustained health home‐visiting program that supports children and families from pregnancy through to two years of age.
An opportunity has become available for an experienced Physiotherapist who is keen to take on a part time position (1 day per week) based at St Marys Community Health Centre.
In this role,



you will provide specialist Physiotherapy services to vulnerable families participating in the SNF program.
You will deliver high‐level clinical consultation, advocacy, education and support to the SNF team, including individual clinician consultation and collaborative contributions to team case conferences.
You will also bring advanced clinical expertise to case planning and case management as part of a multidisciplinary health team, actively participating in case reviews and discussion meetings.

What you will bring to the role
Degree in Physiotherapy and current registration with AHPRA as a Physiotherapist.
Extensive experience in paediatric clinical services, including: Assessment, planning and evaluating Physiotherapy interventions in working with children particularly aged 0-5 years of age presenting with a variety of complex developmental difficulties/diagnoses.
Demonstrated capacity to provide clinical consultation in partnership with the child and family health nurse and facilitate case review, in the context of family centred casework within a community based multidisciplinary team
Demonstrated ability to provide information, advice and education to clients, carers and other professionals, including: Anticipatory guidance, treatment intervention, and management strategies to facilitate progress and promote optimal development of the child.
Demonstrated high level verbal and written communication skills, computer competence and ability to use electronic medical records, word processing and databases.
Demonstrated ability to develop and maintain co-operative professional and therapeutic relationships with clients, government and NGO agencies through the use of well-developed interpersonal and conflict resolution skills
Current Driver's Licence and willingness to use for work purpose travel.
Ability to travel within the LHD in accordance with the position.
