DPV Health invites applications for a Healthy Mothers Healthy Babies Case Manager to provide high‐quality assessment, care coordination and education to eligible clients in the community, supporting self‐management during pregnancy and after birth.You'll work with a multidisciplinary team, CALD and disability clients, travel between sites, and undertake checks required by policy.
A tertiary qualification such as a Bachelor of Social Work or Midwifery is essential.
