You will assess, refer, and support re‐enablement for clients, working closely with nursing and allied health teams.What you'll bringExcellent time management, organisational and communication skills.Compassionate, adaptable and positive approach to patient care.Ability to nurture respectful relationships and collaborate within a team.Experience in service delivery to clients living in the community (desirable).
Experience in nursing or allied health (desirable).
