About the role:
We are seeking experienced, compassionate and reliable Disability/Community Support Worker to provide high-quality, person-centred care to our participants.
As a Community Support Worker, you will work alongside our participants to support them at home and in the community in meaningful activities.
Support is delivered in a way that is responsive, respectful, and empowering.
Key Responsibilities
· Deliver person-centred care and daily living support aligned with participants goals.
· Support daily living activities (ADLs) and routines.
· Assist participants with personal care, domestic assistance, mobility, and medication support.




· Promote social inclusion and community participation activities.
· Implement structured supports, and behavioural strategies.
· Work with the Behavioural plans of participants to reach their goals.
· Maintain accurate documentation, incident reports, and progress notes in compliance with organisational policies and procedures.
Uphold workplace health and safety requirements at all times.
