Primary Objective: Supports and provides team-oriented, appropriate, proficient, coordinated direct and indirect clinical care for complex client presentations, including working with and alongside less experienced nursing and midwifery staff, within the specialty area. The Discharge Planner works within the multi-disciplinary health care team, and with external care providers to facilitate protected and timely discharges for Katherine Hospital patients. The role provides coordinated support for patients to discharge safely into their home setting.
