Better Health Network is seeking a Case Manager to join the Complex Care Team in the Community Building Program.
The role is site-based at Park Towers, South Melbourne, delivering trauma-informed, person-centred case management to residents with complex needs.
You will support timely access to immediate supports and longer-term system navigation (NDIS, My Aged Care) to promote stability, wellbeing and independence.
