Lifestyle Solutions NSW is seeking a dedicated practitioner to support children and young people in Out-of-Home Care through case management, therapeutic planning and frontline coaching.

The role covers two functions: 18 hours per week with frontline staff in homes and 20 hours per week on case work, planning, documentation and stakeholder engagement, with a focus on trauma-informed, culturally secure practices.
