IRT Australia is seeking a Home Care Partner to work closely with customers to enhance their quality of life, enabling them to remain at home while meeting their care needs.
You will provide case management, assessments, and coordinate services to support independence.
The role can be located in the ACT, Far South Coast, Illawarra or South East Sydney/South West Sydney regions.
This is a hybrid position requiring 3 days at the nearest IRT office and 2 days at home.
