About the Role:

Join the multidisciplinary team at Clare Holland House, where Social Workers provide specialised palliative care services across inpatient, home-based and outpatient settings.

Working closely with patients, carers, referrers, health professionals and multidisciplinary teams, you will contribute to the delivery of safe, high-quality, patient-centred care.

This position supports the delivery of Social Work services across Clare Holland House and its specialist palliative care programs. You will play an important role in supporting high-quality care and service delivery within this specialised clinical environment.
