Support at Home Clinical Care Partner Lumia Care | Dulwich, SA

Lumia Care is seeking an experienced and compassionate Clinical Care Partner to join our in-home aged care team in Dulwich, SA.
This is a key role supporting older Australians to remain independent, protected, and respected in their own homes.

About the Role

As a Clinical Care Partner, you will work autonomously in the community while collaborating with a dedicated and supportive team. You will coordinate personalised in-home care for participants funded under the Australian Government's Support at Home program. This role combines clinical expertise, care coordination, and stakeholder engagement to ensure high-quality, compliant, and person-centred service delivery.

Key Responsibilities

- Conduct home visits to assess participant needs, including safety and mobility assessments, and develop individualised care plans
- Build strong relationships with participants, families, and health professionals to deliver tailored, outcome-focused care
- Coordinate services, manage handovers, and ensure care plans, budgets, and supports are accurately documented and delivered
- Maintain compliance with the Aged Care Act 2024, relevant standards, and internal policies
- Respond to participant feedback and complaints, ensuring timely resolution and positive outcomes
- Support continuous improvement initiatives, including quality assurance, policy development,



and accreditation activities
- Attend the office weekly to collaborate with the team and contribute to operational alignment

What You Bring

- Registered or Enrolled Nurse with minimum 3 years' experience in aged care, community, or health services
- Current AHPRA registration and NSW Driver's Licence
- Strong understanding of Commonwealth in-home aged care programs, legislation, and standards
- Well-developed clinical judgement and stakeholder management skills
- Confidence using multiple systems, portals, and Microsoft Office
- A genuine commitment to delivering high-quality, participant-centred care
