Overview
Community Care Coordinator — The Cottage
Employer: St Vincent's Health Australia. Location: Fitzroy, Melbourne, VIC (CBD at doorstep). This role sits within the SVHM Healthcare for the Homeless program (ALERT) and the Cottage, providing proactive care coordination for clients with complex psycho-social and healthcare needs.
Responsibilities
- Maximise patient functional independence through assessment, planning, implementation and evaluation of care.
- Conduct holistic assessments in collaboration with clients using motivational interviewing and self-management approaches.
- Identify and access multidisciplinary and specialist supports to address client issues.
- Facilitate client-generated goals that are realistic and time-limited; coordinate care with clients, carers and families.
- Develop collaborative care plans with the health care team; ensure linkages with hospital and community services.
- Communicate effectively with team members, stakeholders and external service providers; advocate for clients as needed.
- Manage documentation, risk assessment and escalation; ensure appropriate use of funding and resources.
- Provide discipline-specific clinical management, discharge planning and community liaison for Cottage patients.
- Monitor health outcomes and plan transitions from the program with safe long-term community supports in place.
- Contribute to quality initiatives, service development and team processes; meet Activity Based Funding targets.
Qualifications and Experience
- Bachelor or Master’s Degree in Nursing
- Registration with AHPRA
- Completion of basic life support training/CPR/First Aid
- Minimum of 3 years post-registration experience
- Current Victorian Driver’s Licence

Incumbent Obligations
- General: comply with SVHM policies, uphold Code of Conduct, maintain confidentiality, participate in performance reviews, adapt to operational needs.
- Clinical Quality and Safety: maintain registration/indemnity, practice within scope, complete competencies, engage in risk management and quality improvement.
- Person-Centred Care: provide accessible information, support informed decision-making, manage discharge planning for best client journey.
- Health and Safety: comply with policies, complete required training, orientation within 3 months.
Selection Criteria
- Essential: Eligible for AHPRA registration; minimum 3 years experience in relevant acute/community settings.
- Other Essential: Knowledge of care coordination frameworks, ability to work autonomously and in a multidisciplinary team; experience negotiating solutions and developing program activities; strong communication and organizational skills; current Victorian Driver’s Licence.
- Non-Essential: Specialised skills in homelessness, substance use, mental health, aged care, or chronic disease.
