About the Role
We are looking for a Program Coordinator with solid administration, coordination and Home Care experience to support the day-to-day management of referrals for the Illawarra Transitional Aged Care Program (ITACS) and Shoalhaven Transitional Aged Care Program (STACS). Reporting to the Senior Care Partner & Programs Manager, the successful applicant will play a key administrative role in ensuring TACP customers are set up accurately, services are coordinated efficiently, and documentation supports high-quality Home Care delivery aligned to individual goals.
This role is based in Wollongong.
Your responsibilities will include:
- Receive, review and process TACP referrals across ITACS and STACS, ensuring all administrative requirements are completed accurately and in a timely manner.
- Review referral information for completeness and accuracy, following up missing details to support efficient Home Care service delivery.
- Coordinate new customer administration with the Service Coordination team, including accurate customer setup, records management and data entry in AlayaCare.
- Maintain accurate customer documentation, records and program information in line with Home Care requirements, the Aged Care Quality Standards and IRT processes.
- Support service scheduling administration, including urgent or last-minute requests from Area Health, ensuring communication is clear and records are updated.
- Build and maintain effective working relationships with Area Health representatives, case managers, clinical teams and internal Home Care stakeholders.
- Act as a key administrative contact for Area Health TACP Case Managers/Coordinators regarding customer information, service coordination and program administration.
- Attend Area Health case conferences and ensure relevant actions, updates and administrative follow-up tasks are documented and progressed.
To Be Successful You Will Have
- Previous experience in a busy administration, coordination or customer support role, preferably within Home Care, aged care or community services.
- Strong understanding of Home Care service delivery, including customer onboarding, referrals, scheduling, documentation and stakeholder communication.
- Ability to work in a fast-paced administrative environment while maintaining robust attention to detail,
accuracy and follow-through.
- Understanding of the Aged Care Quality Standards and their application to Home Care documentation, service coordination and customer records.
- Demonstrated professional relationship-building skills, with the ability to manage expectations and communicate confidently with internal teams and external stakeholders.
- Ability to interpret and apply policies, procedures, program guidelines and administrative processes accurately.
- Robust written and verbal communication skills, with well-developed organisational, prioritisation and record-keeping skills.
- Demonstrated proficiency using computer systems, customer management platforms and Microsoft Office programs. AlayaCare experience is highly regarded.
- Desirable: experience with TACP, Home Care Packages, CHSP or similar community aged care programs, and/or relevant administration qualifications.
