Senior Helpers Sunshine Coast is looking for an experienced and caring In-Home Care Partner to provide care services to our Aged Care clients. The role of Care Partner is to provide quality care, support clinical management and contribute to the assessment, development and implementation of care plans for Senior Helper’s clients. You will provide a varying range of services based on the client's care needs, your responsibilities in this role will be:
·Complete holistic client and environmental assessments, documenting observations, evaluations, and care planning decisions.
·Apply professional assessment skills to identify physical, mental, and psychosocial needs using a multi-disciplinary approach.
·Develop and maintain personalised, person-centred care plans aligned with identified needs and client goals.
·Review and update care plans regularly to ensure they remain consistent with changing client needs.
·Arrange and monitor services such as allied health, home support, and equipment provision as directed.
·Conduct regular client check-ins to assess wellbeing and promptly report any changes.
·Liaise effectively with clients, families, support workers, and external service providers to ensure continuity of care.
·Empower clients to maintain independence and informed choice through person-centred care approaches.
·Manage the client’s care budget,
ensuring services are delivered within program guidelines and funding constraints.
·Evaluate client outcomes and care quality on a regular basis, identifying areas for improvement.
·Ensure all clinical and assistive equipment is fit for purpose and properly maintained, with accurate records kept.
· Collaborate with management to implement and refine operational procedures that enhance quality client care.
·Triage and first aid, including emergency procedures
·Medication administration and monitoring
·Wound and drain management
·Blood sugar monitoring and diabetes education
·Palliative and end-of-life care, including family support
Job Type: Permanent or Casual
Pay: $46.36-$64.01 per hour (depending on experience)

Licence/Certification:
·Diploma in Nursing, Social Work (or equivalent)
·Minimum of 10 year’s practice experience
·Current registration with AHPRA
·CPR Certificate (HLTAID009) or Equivalent
·Current unrestricted Queensland C Class Driver’s licence
·Current Working with Vulnerable Persons’ Card
Strong background in mental health desirable
