We'reseeking a Wellbeing Facilitator to support adults throughindividual sessions and group activities, using strength-based,person-centred approaches to co-design holistic and tailoredrecovery plans. You'll collaborate with care teams and stakeholdersto deliver proactive, recovery-oritentated services that make alasting impact.

What will a typical day look like?

- Establish apositive team-oriented working relationship with peopleexperiencing severe and persistent mental health challenges,their chosen care team, and key professional stakeholders tocollectively work toward an identified preferredfuture.
- Provide co-designedindividual and group-based recovery-oriented services that aretimely, proactive, person-centred and strengthsbased.
- Work alongsideparticipants and their clinical case managers, nominated healthprofessionals and/or partner organisations, to deliver the bestpossible comprehensive service and outcomes.
- Maintain participants' records to a high standard, compliant withstatutory obligations and AnglicareCQ's policies.
- Co-designindividualised care,



safety and recovery plans with peopleincluding implementation and review that include goals andaspirations for recovery.
- Develop strongpartnerships with key stakeholders.
- Encourageself-agency from service commencement totransition.
- Completion ofassessment, planning and other documentation required by AnglicareCQ and funding bodies.

MandatoryRequirements:

- Diploma levelqualifications (or higher) in a Mental Health relatedfield.

- Astrong communicator, able to quickly build rapport andinfluence across a diverse range of people.

- Ability to obtain aBlue Card (working with children check positive notice orexemption)
- Ability to obtain aYellow Card (disability services positive notice or exemptionnotice)
