Ability Action Australia is growing — and we’re looking for a Behaviour Support Practitioner to join our multi-disciplinary team on the Sunshine Coast. The successful candidate will be providing supports to NDIS participants living close to the Glasshouse Mountains area.
Responsibilities Will Include
- Assisting NDIS participants, along with their families ∨ carers, to manage challenging behaviours to improve their quality of life, and develop new skills
- You will complete thorough assessments to understand behaviour triggers, identify risks & consider specific strategies to improve behaviour outcomes
- You will develop Behaviour Support Plans and provide support to families ∨ carers, for consistent implementation
- Maintain a caseload with ongoing monitoring, support and make adjustments where required.
- Develop meaningful goals and practical strategies utilising a positive behaviour support framework, with a commitment to eliminate restrictive practices, in the home and community
- Ensuring individual plans reflect our NDIS participants' goals, passions and strengths, as well as ensuring they get more out of life
- Build the knowledge,
capacity and strengths of those who support participants in their lives - family, carers or friends
- Act as an advocate and role model for best practice
- Working collaboratively within a multidisciplinary team
