The Salvation Army is seeking a Case Manager for the Homeless Outreach Program in Nunawading, VIC. This permanent part time role (22.8 hours per week) focuses on outreach, holistic case management, housing and health referrals, and client advocacy to improve wellbeing and sustainable housing options.

Reporting to the Team Leader, you will work with people experiencing homelessness or at risk, developing tailored plans and connecting them to essential services.
