About the Role:
As a Care Coordinator, you'll play a vital role in supporting the delivery of high-quality care by ensuring client information, care plans and service records are accurate, up to date and compliant. Working closely with Care Partners and Team Leaders, you'll coordinate onboarding, maintain documentation, and support the smooth day-to-day delivery of services.
Key Responsibilities
- Coordinate care services to ensure clients receive timely, high-quality support.
- Maintain accurate client records, care plans and service documentation.
- Support workforce onboarding and ensure compliance requirements are met.
- Monitor service delivery and proactively resolve or escalate issues.
- Update client and workforce information across internal systems.
- Collaborate with Care Partners, Team Leaders and key stakeholders.
- Deliver exceptional customer service while managing competing priorities.
- Contribute to a culture of quality, compliance and continuous improvement.
About You
- You're a highly organized and adaptable professional who thrives in a fast-paced, changing environment. You have strong attention to detail, excellent communication skills and the ability to juggle competing priorities while maintaining accuracy. You enjoy working collaboratively, take initiative, and are committed to delivering exceptional service that makes a genuine difference to clients and your team.
Desirable
- Certificate IV or Diploma in Business Administration, Community Services, Health Administration or a related field.
- Minimum of 1 year experience in a coordination, administration or scheduling role, preferably within the health, aged care or community services sector.
- Experience using client management or rostering systems, with the ability to maintain accurate records in a fast-paced environment.
