The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you a dynamic physiotherapist with a passion for aged care and slow stream rehabilitation? Join our dedicated team delivering high-quality physiotherapy services across a range of community-based programs,
including: - Transitional Aged Care Program (TACP) - Nepean Post Acute Community Care (NPACC) - Commonwealth Home Support Program (CHSP).
In this role, you’ll work closely with clients transitioning through various stages of care, helping them regain independence and improve quality of life in their own homes and our Community Health Centre's. We're looking for someone who thrives in a collaborative environment, brings energy and empathy to their practice, and is committed to delivering high quality, evidence-based interventions
The ideal candidate is a qualified physiotherapist who is passionate about aged care and rehabilitation, demonstrates strong clinical reasoning and communication abilities, and is committed to providing compassionate, client-centred care.

What you will bring to the role
- Tertiary qualifications in Physiotherapy and Current National Physiotherapy Registration with the Australian Health Practitioner Regulation Authority (AHPRA).
- Ability to provide high-quality clinical services across a broad range of clinical areas.
- Demonstrated effective written and verbal communication skills.
- Demonstrated effective time management skills.
- Demonstrated commitment to professional development and demonstrated knowledge of Quality Improvement & Evidence Based Practice.
- Demonstrated high level communication and interpersonal skills within a health environment, with experience working in multidisciplinary team settings.
- Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
