Community Health Nurse – Homecare Services

Better Health Network (BHN) is seeking a passionate and experienced Community Health Nurse to join our Homecare Services team. In this rewarding role, you'll provide person-centred nursing care to people living in the community, helping them maintain their independence, wellbeing and quality of life.

About the role
As a Community Health Nurse, you'll work with adults who have a range of health and wellbeing needs across community-based programs including Support at Home, Post Acute Care and other community services. You'll provide clinical nursing care, care coordination and health education in clients' homes and other community settings.
Your responsibilities will include:
- Conducting comprehensive nursing assessments and developing person-centred care plans.
- Delivering clinical care including wound management, medication support, continence care and chronic disease management.
- Coordinating services and navigating referral pathways to ensure clients receive integrated care.
- Managing a caseload and maintaining accurate clinical documentation.
- Working collaboratively with clients, carers, allied health professionals, hospitals, GPs and other community service providers.
- Supporting quality improvement initiatives and delivering high-quality, person-centred outcomes.
About you
You'll be an experienced Registered Nurse who enjoys working autonomously while building strong relationships with clients and multidisciplinary teams.
To be successful, you'll bring:
- Registration as a Registered Nurse with AHPRA.
- At least three years' post-graduate nursing experience, ideally within community, outreach or Hospital in the Home settings.
- Experience providing clinical nursing care in community-based environments.
- Strong assessment, care planning and risk management skills.
- Knowledge of aged care, community health services and referral pathways.
- Excellent communication, care coordination and teamwork skills.
- A current Victorian Driver Licence.
Desirable:
- Post Acute Care experience.
- Wound management competency.

Location
This role is primarily based at the Victorian Pride Centre, St Kilda, with travel required across community settings and other BHN locations as needed.
