JOB DESCRIPTION
Total 247 Care Pty Ltd is seeking a permanent full-time Community Worker to provide community-based and in-home support services to clients in accordance with individualised care and support plans.
Key Responsibilities:
- Provide direct support to clients in their homes and community settings
- Assist clients with daily living activities in accordance with individual service plans
- Support community participation, employment and social engagement
- Monitor client wellbeing and report concerns to management
- Maintain accurate client documentation and case notes
- Liaise with families, carers and multidisciplinary teams
- Assist in implementing and reviewing client support and behaviour programs
- Ensure compliance with NDIS standards, policies and procedures
